Team IMPACT connects children faced with serious or chronic illness to college athletic teams, providing a family-centered support system that helps with the emotional trauma and social isolation that follows a life-altering diagnosis. PROGRAM OUTCOMES: Parents report that their children gain improved: • social skills • confidence and independence • coping and resiliency • interest in physical activity
ELIGIBILITY: For children (ages 5-16 years old) struggling with the social and emotional trauma that comes with fighting a life-altering diagnosis.
PROGRAM REQUIREMENTS:
• regular phone calls with Case Manager for ongoing support (1-3 times/month)
• ability to use an internet-based platform
• child's ability to communicate effectively via online platform
