I understand that I must contact camp to submit an invoice for camp registration fee via email to sbwis@sbwis.org.

I understand that incomplete submissions will not be considered.

I understand that if my request is for attending UW-Whitewater Camp, SBWIS will send me a code to enter in their system when completing online registration.

Help us say thank you to our donors. *

We request that campership fund recipients share photo(s) and a note about their camp experience. We will use these to thank our donors. When sharing with donors, SBWIS will only include camper's first name, age, city of residence, camp location, summary of your note and photo. Please email photos and notes to Kathy Mohar at sbwis@sbwis.org or mail to our office.

I understand that it is my responsibility to send a thank you note and photos to SBWIS expressing how the camper benefited from their experiences.

I give permission to SBWIS to use my photo and the testimony that I provide on their website and Facebook page.

I understand that if I don't send a thank you and photos, this could impact future requests for Campership funding.
