Scholarship submission

Scholarship for higher education in memory of Mary Ann Potts, Ph.D.

Congratulations to Logan for being our 2023 Scholarship for Higher Education recipient.

SchOLARSHIP FOR HIGHER EDUCATION - ACHIEVE YOUR DREAMS!

We admire the drive and tenacity it takes for our members to pursue higher education. SBWIS is proud to offer this financial assistance due to the generous donation from the Potts family in memory of Mary Ann Potts, Ph.D. who had spina bifida and achieved her dreams! This fund was established in 1996 by the Potts/Weaver family.

Criteria: Applicants must be a Wisconsin resident with primary diagnosis of spina bifida. Applicants must be enrolled in an accredited institution of higher education (University, College, Associate Degree Program, Vocational Training Program) for the current academic class period. Applications are reviewed and awards are determined by our Board of Directors. Students who have previously been awarded this scholarship are eligible to apply again with an updated personal statement and application materials. Scholarship awards will be paid directly to the institution of higher education. Applicants will be notified via email. Upon notification, awarded applicants must provide SBWIS with one photo (jpeg format) to use on our website, social media and to share with our donors and a thank you letter for us to share with donors.

We want to learn about your dreams and future plans. Your application must communicate your goals and help us understand how you tackle life and overcome obstacles to reach these goals. Good luck!

Application steps include: 1. Complete this online submission form (be sure to include the appropriate office and identifying information for where the scholarship award money should be sent).

2. Post mail or scan/email the additional required documents. If you choose to post mail the documents, be sure to allow plenty of time for mail delivery. Mail can not require a signature and it must be received by the deadline (not postmarked). Scanned/emailed documents are secure and confidential.

Documents can not be dropped off at the SBWIS office due to an unfair advantage to those who live in the Metro-Milwaukee area. As a statewide organization, the submission process and deadline must be the same for everyone.

3. Meet the deadline of midnight April 28, 2024. Application and all supporting documents must be received by this deadline (no exceptions).
